Established in 1997, Aabcor, Inc. has been a trusted private case management agency dedicated to individuals with complex health needs. Since contracting with the State of Illinois Department of Human Services/Office of Rehabilitation Services in 1999, we have managed the TBI, Aging, and General Disabilities Waivers, helping over 5,000 individuals and families receive essential home-based services. Proudly, we are a certified BEP company and a WMBE (Woman Minority Business Enterprise), committed to delivering compassionate care and support.

Our Team

At Aabcor, Inc., we are a multi-disciplinary team of licensed social workers and healthcare providers committed to improving the lives of individuals with complex health needs. With deep expertise in Illinois waiver programs and managed care organizations, our team receives annual training led by DHS/ORS and certification through the ORS department, ensuring we deliver the highest quality of care and support for our clients.

What We Do

  • Our Referral Intake and Community Liaisons collaborate with rehabilitation hospitals, healthcare facilities, physicians, psychologists, neuropsychologists, and other brain injury professionals. They provide immediate follow-up to referrals and ensure that our case managers visit survivors and their families promptly after discharge from facilities. In addition, they coordinate with other medical professionals to review survivors’ medical records and recommend tailored care plans that meet each individual’s unique needs.

  • Our Case Managers bring the knowledge, skills, and experience necessary to provide high-quality services to individuals with multiple complex needs. With baccalaureate and graduate degrees in social work, psychology, counseling, and related fields, they develop personalized care plans and act as vital links to resources, services, and opportunities. By building strong, trusting relationships with survivors and their families, our case managers contribute significantly to improving the quality of life for those in their care.

  • Our Caregiver Support Team is available to address problems and concerns as they arise, ensuring that solutions are coordinated quickly. They also provide ongoing follow-up and support to survivors and their families, ensuring that their evolving needs are continuously met, offering peace of mind and long-term stability.

  • Behind the scenes, our Human Resources, Information Systems, and Quality Improvement teams ensure the smooth operation of Aabcor, Inc. From recruiting and training qualified staff to maintaining essential records and computer systems, these teams work to ensure that our services consistently meet the highest quality standards. They are the foundation of our organization's seamless operations and continued improvement.
