NAMI (Support Groups)
NAMI Chicago provides comprehensive support for mental health through a variety of services aimed at promoting wellness in the community. Their Helpline, available seven days a week, connects individuals, families, and providers with resources for mental health support, legal assistance, and housing.
Beyond phone support, NAMI Chicago offers in-person advocacy, peer-led support groups, and educational sessions to foster community and recovery. Their training and education programs empower individuals and organizations to recognize mental health signs, provide de-escalation techniques, and promote workplace wellness, serving diverse audiences like first responders, students, and corporations.
NAMI Chicago also engages in advocacy efforts to reform the mental health system, working with policymakers to improve access to services. Their outreach team reaches individuals across Chicago, supporting those transitioning from residential facilities to independent living, fostering a sense of hope and recovery.
PA Timesheet Schedule
The forms within the page linked above are necessary to begin working as an individual provider for a customer under the Division of Rehabilitation Services (DRS) Home Services Program (HSP). These forms are employment applications for an HSP Customer, must be completed in their entirety, and must be returned to the customer's local DRS office. If the customer is also enrolled in a Managed Care Organization (MCO), these documents will still be turned in to the customer's local DRS office.
